폴리우리아(/ˌp polyliˈjʊriə/)는 과도하거나 비정상적으로 큰 소변의 생성이나 통로(성인의 경우 24시간 동안 2.5 L[1] 또는 3 L 이상[6])이다. 소변의 생성과 통로의 증가는 이뇨증이라고도 할 수 있다.[7][8] 폴리우리아는 다른 것 없이 하나를 가질 수 있지만 폴리디피스와 함께 나타나는 경우가 많고(갈증 증가) 후자는 원인이나 결과일 수도 있다. 일차 다지증은 다지증을 유발할 수 있다.[9] 다뇨증은 보통 다른 질환의 증상이나 징후(그 자체로는 질병이 아님)로 보이나, 적어도 근본 원인이 명확하지 않을 때는 장애로 분류할 수 있다.[citation needed]

원인들

성인과 어린이 모두에게 다뇨증의 가장 흔한 원인은 조절되지 않은 당뇨병으로 [6]포도당 수치가 너무 높아 소변에서 포도당이 배설될 때 삼투성 이뇨를 일으킨다. 물은 포도당 농도를 수동적으로 따르므로 소변 출력이 비정상적으로 높다. 당뇨병이 없는 경우 부신피질종양, 1차 다지피디아(과다수액음주), 중앙당뇨병인시피두스, 신혈당뇨인시피두스로 인한 알도스테론 분비가 감소하는 것이 가장 흔한 원인이다.[6] 폴리우리아는 또한 이뇨제, 카페인, 에탄올과 같은 다양한 화학 물질 때문일 수 있다. 또한 심실초음맥후, 심방세동, 출산, 요로내 장애물 제거 시에도 발생할 수 있다. 이뇨증은 바소프레신, 안지오텐신 2세, 알도스테론 같은 항이뇨제에 의해 조절된다. 냉이뇨는 추위에 노출되면 소변생산이 증가하는 것으로, 이 역시 부분적으로 몰입이뇨를 설명한다. 고고도 이뇨는 고도 3천 m 이상의 고도에서 발생하며, 고도에 적응하는 바람직한 지표다. 높은 고도에 잘 적응하고 있는 산악인들은 이런 종류의 이뇨작용을 경험한다. 적절한 수액 섭취가 있더라도 소변을 적게 내는 사람들은 아마도 고도에 잘 적응하지 못하고 있을 것이다.[medical citation needed]

메커니즘

삼투성의 경우 다뇨증은 유속과 속도가 낮은 원위 네프론에서 유량을 증가시킨다. 원위 네프론에서 발생하는 상당한 압력 상승은 특히 피질 수집 덕트에서 발생한다. 2008년의 한 연구는 고글리케인과 삼투성 다뇨증이 궁극적으로 당뇨병성 신혈증에 역할을 한다는 가설을 제시한다.[39]

치료

다뇨증의 원인에 따라 적절한 치료를 받아야 한다. NICE에 따르면 데스모프레신은 다른 치료법이 실패했을 [5]경우 당뇨병에 의해 생길 수 있는 야행성 다뇨증이라고 볼 수 있다. 권고안에는 고려 기준에 맞는 연구가 없었다.[40]
